In my utopia I would require, at minimum, a panel of psychologists to sign off on and a waiting period of 6 months for any cosmetic procedure that isn't correcting anything 'real' (ie deformities, scarring, aging, gender dysphoria, mastectomies, etc; not "my nose isn't Aryan enough")

I'm talking about random NPCs. The PCs being free agents who go around genociding goblins is normally the thing that makes them unique. I don't think there's a single NPC in BG3 who's identifiably a peasant, which is insane

BG3 is a good example of what I'm thinking of. There's no real sense of what place of most of the NPCs have in the social hierarchy. Everyone's seemingly a modern middle class professional with the complete independence to go anywhere and do whatever they want

My point is just that it's annoying when medieval fantasy settings have the superficial pretense of GoT-esque "medieval" pseudorealism with intrigue and tyrannical nobles and so on, but fail to support that. Basically the exact opposite kind of fantasy you're talking about

that's what I meant, like you still have the standard pseudo medieval technological base, but widespread magic means the average person gets to chill in the tavern after their shift at the magic sword factory instead of being a subsistence farmer
