This phased in approach means we have time to come up with strategies to limit the damage that will occur and even advocate for future repeal of the law. In other words, we can and will continue to fight like hell.

Reduced fed funding for Medicaid: largest fed funding cuts increase gradually in 2028 for most states, but some cuts will start as soon as 2026

Increased Medicaid eligibility checks: starting in 2027
New Medicaid work reporting requirements for individuals covered under expansion: states that have expanded Medicaid must implement these requirements by April, 2027, though they can seek federal approval to delay this until January 1st, 2029.

Reduced fed funds for SNAP for most states: Fed support for states’ administrative costs reduced fall of 2026, & fed support for SNAP benefits decrease fall 2027 to fall 2029 at the latest.

1/1/2026, all lawfully present immigrant residents with incomes under 100% of the federal poverty level will be ineligible for ACA marketplace coverage. In 2027, DACA recipients, Temporary Protected Status, asylum seekers, & most others without a green card will be ineligible for ACA subsidies.

Changes making enrollment in ACA health plans more difficult: these changes technically take effect immediately, but in practice will roll out over the next year as federal agencies issue rules and guidance. The open enrollment period will be shortened by one month starting this fall.
