but for me the solution was to privilege the individual suffering from less utility over those who might benefit from more utility but are already well-off. Weak negative utilitarianism allows us to internalize the diminishing returns of utility without rejecting utilitarian frameworks outright.

In reference to the chart you shared, I'm curious why you were drawn to egalitarianism rather than weak negative utilitarianism. I also have the same instinct you described, a bias towards the situation where both people are relatively well-off, (1/2)

the regimen might be changed based on blood test results but usually in basic ways that don't necessitate an endocrinologist's input (slightly changing dosages)

negative health effects are rare and easy to identify with rudimentary blood tests through an informed consent healthcare provider. (2/2)

as someone who has helped dozens of trans women access healthcare and been through the ringer myself, this is not very true. most people start off with the same or a similar regimen of hrt (1/2)

they aren't allowed to distribute testosterone, but there's a non-profit in my city that directly gives estrogen and anti-androgens to people without a prescription, and does the bloodwork for free

it is illegal for testosterone, but doesn't seem to be for other forms of HRT
