Dementia presents differently in everyone, and as you said he’s being pumped full of drugs.
I’m not arguing that he’s also got other severe physical problems, but his all-night rage posting is pretty indicative of sundowning. Especially since he falls asleep at high-profile events during the day.

I do know someone with dementia.
He is all the things you say AND he is displaying signs of late-stage dementia. He’s crashing out AND he has dementia. And the cabal he’s surrounded himself with is taking advantage of it.

Not a mental health crisis. He has dementia. He had dementia during his first term (why do you think he claimed Biden had dementia all through the 2020 elections and Biden’s presidency?) and it’s progressed significantly.
