Maybe Ghost starts off as an auditory hallucination while Soap’s mind slowly starts to build up an appearance for him. Finally appearing as a fully formed figure after Soap suffers some sort of traumatic injury

Now I’m wondering how their first meeting would have gone. 🤔 Disregarding mw3, what if Soap first “met” Ghost when they were hunting for Hassan. They don’t meet on the tarmac, but Soap hears a new voice through the comms after he watches the helicopter go down 😮
