I do use modded gear and such a lot more. However Saphyra's appearance gradually had some changes.
Saphyra has a bit more weight on her now plus stretch marks after having kids. She also has starlight freckles that appear when using her abilities or in moments of strong emotions.

Saphyra started out slender (left pic). After starting her adventure as WoL she gained a little bit of muscle. Then she gained weight & stretch marks (right pic) when she was carrying her children and not all the weight came off after. So after 2 preganancies (one with twins) she's got some pudge.
