They only financially make sense with one consultant supervising 3 AAs. I haven't worked at a hospital that could regularly produce 6 lists a day with only ASA 1/2 Patients and put them in theatres near enough to each other to make that work.

I start most people on 0.5 to see what happens. If they stop talking and close their eyes then about 2 is a good starting point. If they don't even notice it's probably 3.5+. I use a decent amount of fentanyl bolus for most cases and run remi at 2-3 as propofol sparing adjunct.

I moved to eleveld 18 months ago. I really like it. I find numbers needed more consistent between patients. Did find they were getting a little light after an hour and a half though. But always use sedline. Try to always use it for remi too.

Seen a few families in ITU a week later seriously regretting what the pushed for "they wouldn't have wanted THIS". It's a failure of us to explain the misery and futility of a lot of what we can do.
