Do you cover Lady May Baird? Pioneer of access to contraception and major figure in improving public health in Aberdeen from the late 1930s on. Doesn't even rate a Wikipedia entry!

McKeown works better for other respiratory and GI infections. Shouldn't simply be read across for period since 1950. A big critical literature. Good value are

McKeown argues that medical science (inc vaccination) made only a small contribution to mortality decline 1850-1960 compared with environmental improvements. (He understated the role of public health)
Diptheria is a hybrid case because antitoxin brings death rates down quite a lot from about 1900.
