Yes, each scene revolves around a single challenge, and ends either in success (moving closer towards your goal), or a failure and a setback (moving farther away from your goal).

If the protagonist faces another challenge, that's a different scene.

Nice post, thanks for sharing! I do something similar:
> Setup: In a [location], I encounter a [character/event/obstacle].
> Objective: I want [x], but [y] makes it difficult.
> Action: The action I take to get what I want.
> Resolution: Die roll result, description of the outcome.
