If it looks biochemical:
Prioritize physiology: breathing mechanics/CO₂ literacy, sleep/recovery, reflux care, graded movement, stress load.

If it looks conditioned:
Start with psychoeducation → thought work → gradual exposure to sensations/contexts that trigger fear.

“Yes” to both → often a conditioned pattern (avoidance learning).

If spikes correlate with load/physiology (sleep, stress, reflux, subtle hyperventilation), consider a biochemical track.

Two questions:

- Are your attacks recurrent and unexpected?
- Have you started worrying about more attacks or changed your behavior to avoid places/sensations?

Conditioned:
Fear attaches to sensations/places: heart racing, breath shifts, trains, restaurants, tunnels.

Avoidance grows; life shrinks.

Biochemical:
More tied to physiology or load: poor sleep, prolonged stress, subtle over-breathing, reflux.

Often feels like “it came out of nowhere” and repeats during high-stress periods.

Why this matters:

Different causes → different tools.

A body-driven spike won’t respond well to the same strategies as a learning-driven fear loop.

- Biochemical (body-driven: breathing/CO₂, reflux, chronic stress)
- Conditioned (learning-driven: interoceptive/classical conditioning)
