APIPPRA also followed patients for 12 months AFTER STOPPING ABATACEPT, and treated patients still had benefit!

Said another way: it's possible ABA in the pre-clinical phase PREVENTED FULL RA.

As the kids say: Big, if true.

Quick reminder of how abatacept works: it's CTLA4 bound to the Fc of an IgG.

CTLA4 is a molecule we already have that blocks CD28-CD80/86 (B7) interactions.

CTLA means T cells Can't Take it Any Longer (Get it? rearrange the first letters?)

This is DIFFERENT than the CD28 co-stimulatory pathway most #rheumatologists are familiar with because we use abatacept (CTLA4-Ig).

CD28-CD80/86 (aka B7) is about APCs ACTIVATING T CELLS

CD40L-CD40 is about T CELLS ACTIVATING OTHER CELLS (like B cells)

So, what is CD40L? It's an important co-stimulatory molecule. Think of it like multi-factor authentication for immune cells. They don't just need a password, they also need a push notification.

For example: CD40L on T cells binds CD40 on B cells, revving them up.

Most importantly, this study is one of the first to demonstrate a strong positive impact on pain, dryness, and fatigue, as defined by high ESSPRI scores.

Cannot overstate how exciting this is!

This trial showed positive results in patients with high systemic disease activity, as defined by high ESSDAI scores.
