FYI: You link to the 2/2011 version, which still mentioned Seasonal Agricultural Worker (SAW). This option was removed in the 12/2022 version. Both versions have issues in Adobe Acrobat, where step 2 only allows 1 checkbox to be selected. (Google chrome and Mac finder preview do work).
