And the vast majority academics, who societies reward with tenure exactly so they can maintain independence and speak to power, are basically mute on all of this. Six figure salaries and jobs for life, but they refuse to even acknowledge the problems, content in their little "most cited" awards.

It is not only ok for you to say when you don't understand something, it's critical. Experts know their limits, almost by definition. Novices get swept up in hype.

We've seen a similar decades-long slide in exactly this direction in Ireland (and I imagine many other countries too).
But once he's are in place, it becomes very hard to convince politicians that is it worthwhile to restore free or cheap education... Even given the popularity of it […]

If the term "Real World" is supposed to be a short cut for saying the results of a study will be more widely applicable, then that's already something experts need to think about regardless of whether the data were collected in an RCT or an electronic health record.
