I don't get it... Why do you say this like it's a problem?

I'm not a big fan of a cut in the middle between thongs and briefs. With the buttplug already forcing the panty in between the butt cheeks, I feel like the cut should be closer to a thong. With the back being a thinner T-shape.
But it's not easy to say without seeing it on a real person.

You can but you have to treat the surface before using it as a mold. Resin might be a good bet but frankly, I'm still experimenting with that

As for where to buy it from, it depends where you live

In most countries, "medical grade" is for finished products and not raw materials. Even with the best materials, if your mold gives your product a porous surface, it may not be "medical" grade.
Look for platinum silicone. Prefer those meant for kitchen gadgets and/or prostheses.
