Methanol is converted to the very toxic formate by ADH and ALDH in the liver. Formate inhibits mito Complex IV causing cellular hypoxia and acidosis. ALDH is the critical step and ethanol/Fomepizole are competitive inhibitors of ALDH, preventing it from acting on MeOH, allowing its renal excretion.
