Agencies can provide "pre-times" to customers with locked in times (to the second) when an ad is due to air. Fill is the thing that buggers this up. Advertisers need to stipulate no ads NOT EVEN FILL to go in that show

An advertiser can, however request that their ads do not air in a show, or in a time slot, or even nowhere near a promo for that show that may run at a different time. A good agency should be aware of the backlash and be prepared to accomodate these wishes. I worked in local radio for years.

Very much depends on how they buy the ads. You can buy by "day part" like Breakfast, Morning, Afternoon, Drive, Evening or stipulate a particular hour or show. It is possible (especially if there's room due to cancellations ) that spots go to "fill" and any commercial is used to fill that gap.
