OVESCO is possible for anastomotic bleeds. This patient with gastric bypass several years ago, had multiple episodes of bleeding since then.
Presented to our unit with melaena and significant anaemia. Treated with OVESCO successfully. I used 11/6t clip but ideally should be 11/6a (atraumatic)

It's only fitiing that I start my #bluesky journey with a post on third space endoscopy.
I feel that there is less recurrence post Z-POEM in the setting of mucosotomy esp in large pouches. Here is the method that I use. You have to be confident of your ability to close if you embark on mucosotomy
