Summerhayes says something about it in "Wild Orchids of Britain", he calls it vegetative multiplication, but does not explain how it works for Dactylorhiza, only that it occurs and he gives some examples. "130 individual plants in the clump!"

Most likely. Harrap on Southern Marsh: "Vegetative reproduction, although far less important, may sometimes produce groups of plants", and on Common Spotted: "Vegetative reproduction is also possible".
