A must read paper

And the end of the conclusion:
'Accordingly, we believe that the time has come to recommend AVR for asymptomatic patients with severe AS. This paradigm shift is anticipated to yield improved patient outcomes, public health, and resource utilization. It is time to act.'

The conclusion: 'In an earlier era when procedural risks were substantially higher than they are today, one of the authors (E.B.) wrote that “operative treatment is the most common cause of sudden death in asymptomatic patients with AS.”10 In the 35 years since that editorial, much has changed.'
