We hope that study findings provide a starting point for understanding #patientsafety incidents within community-based mental health services, including under-recognised harms. Findings should be considered together with the experiences of service users and families to inform interventions.

3️⃣ Beyond mental healthcare-specific incidents, incidents seen in all types of healthcare settings were also described.

For example, documentation errors meant that medications could not be dispensed, resulting in a treatment break. This placed service users at risk of relapse

2️⃣ Under NHS England’s Serious Incident Framework, we’ve seen repeated investigations of similar incidents.

Here we shed light on underexplored ‘moderate’ harms, with Mental Health Act assessment cancellation a key incident described. Outcomes included self-harm, deterioration, risks to others

It was agreed that a broader code of ‘safe care’ is needed, focusing on helping people to live fulfilling lives, rather than just managing risks. Priorities for making care safer included: improved access, smoother transitions, supporting carers, and safer medication practices

A third perspective recognised that mental health providers can both contribute to harm or help prevent it through their actions. Things like cancelling appointments, not following up patient non-attendance, medication errors, or lacking compassion in consultations can negatively impact safety
