From the 2004 All-Clad seconds sale. Back when finding information about it was not exactly easy. So much so that I had newspapers linking my old blog post about it. Now? I get slammed with emails about it regularly.

Looks like Arena is having play issues. Never seen that sleeve (but unclear if it was ever offered), but the stats in the upper right are super glitch.
