Using a prospectively registered methodology, we included and reviewed 99 articles that manipulated the enzymes or characterized DNA methylation at candidate genes, or more recently genome-wide, including in the context of in utero or developmental exposure to drugs.

Overall, we uncover a profound epigenetic sex divergence that mediates convergent biological dysregulation, and highlight opportunities for developing improved therapeutic strategies tailored to sex-specific mechanisms.

These adaptations nevertheless followed common mechanistic principles across sexes, acting at similar gene features & transcription factor binding sites. They converged on overlapping genes & co-expression modules, and partially recapitulated transcriptional signatures of addiction in human brains.

Despite comparable tissue organization (multiplex ISH) and neurophysiological responses to opioids (calcium and dopamine fiber photometry in freely moving mice), epigenetic adaptations occurred at highly sex-specific genomic loci.
