I‘m pretty sure it‘s a continuum. Full dose with best stroke prevention. But eldercare AF taught us, that „tiny dose doac“ still has a huge effect compared to nothing. www.nejm.org/doi/full/10....

Great educational case! The correlation between FFR and resting gradients is ok, but far from perfect (R values ~0,8).
The answer to this case is nicely demonstrated in the paper below: Resting gradients tend to underestimate focal stenosis due to low friction. doi.org/10.1161/CIRC...

I really don‘t like the classification in Type 1/2 MI. It doesn‘t differentiate between demand ischemia and acute coronary obstruction! It differentiales between acute atherothombotic event (T1) and all others (T2). So SCAD and Vasospasm are Type 2 MI although thats clearly not demand ischemia.

Ich finde das Thema Faktor XI-Inhibition schon spannend. Die Hoffnung wäre, dass dadurch selektiv der intrinsische Gerinnungs-Weg gehemmt wird. Theorie: Thrombus im Vorhofohr wird verhindert, nicht aber Hämostase nach Gefässverletzung. Niedrigere Blutungsneigung wäre dann schon ein Mehrwert.
