If you read the plan name it’s a community plan meaning Medicaid … this is regulated by each state and they dictate what is covered. Not making excuses but this is on Arizona .

Truly horrific! I agree insurance sucks and I work in insurance. We deny coverage to me that is silly but it’s not always known that if you have a self funded plan from your employer the insurance company doesn’t deny your employer does based on the benefit they purchased.
