we encountered a similarly dramatic case of disseminated sporotrichosis during my ID training.

he was thought to have sarcoid and received glucocorticoids. only later did a dx of hairy cell leukemia become apparent.

i saw an unforgettable case as a pgy1. we actually aspirated a lesion in ED and found GNB on gram stain.

One cannot overstate the value of a detailed skin examination in the diagnosis of systemic disorders, including infectious diseases. This precept was demonstrated in the present case.

there are times when the deep dive is warranted, but including every detail in a case presentation (where most of the details are already in the note) is often superfluous.

and yes, there is skill in being able to distill a case into its core features.
