Our volunteers' experiences of accessing the healthcare system have been captured in several short films, an important resource for the IMPROVE programme (Intervention to optiMise Palliative caRe for peOple with liVed Experience of homelessness).

Last week our lived experience team contributed to NIHR funded research led by @pathwayuk.bsky.social Fellows, @brionyhudson.bsky.social, Dr Caroline Shulman and @inclusivepeolc.bsky.social. The realist evaluation of IMPROVE will optimise palliative care for people experiencing homelessness.

We hope they inspire services tackling health inequalities in their local areas.

Along with @itsgroundswell.bsky.social and @homelesslink.bsky.social, we are publishing today a new case study resource showcasing best practice to address these health inequalities, highlighting how much effective collaboration across the health and homelessness sectors can achieve.

Today's figures confirm once again that homelessness is a health catastrophe.

We look forward to the Government making the right choices in the forthcoming homelessness strategy.

The achievements of the Pathway hospital teams highlight the critical role of the NHS in ending homelessness.

Alex Bax, @pathwayuk.bsky.social's CEO, gave a presentation on the hospital-based Pathway Partnership Programme teams who have a track record of ending and preventing homelessness.

These multidisciplinary, expert teams achieved a 62% reduction in patients returning to rough sleeping in 24/5.

✔️ Making housing status a mandatory field in NHS data is a simple, low-cost, high-impact solution.

The average age of death for women and men facing homelessness is 43 and 45. They suffer from preventable and treatable conditions like asthma and heart disease. 

❗Yet, there is still no systematic way to accurately record or identify patients facing homelessness in the NHS.
