5/ Why this matters

One of the clearest explanations of PEM to date. It shows that even gentle exercise can trigger clot fragmentation and inflammation. This should be another wake-up call for those still pushing blanket Graded Exercise Therapy.

4/ What they found

- Large microclots fragmented into smaller ones
- Inflammation markers surged post-exercise
- Oxygen delivery got worse on Day2

3/ How was it done?

Participants did two submaximal cardiopulmonary exercise tests (CPETs), a common way to assess exercise tolerance. They chose participants who had a low risk of PEM. (imagine how bad this is for those with severe PEM)

2/ What did they do?

46 people with Long COVID (low risk of PEM) did two gentle exercise tests, 24hrs apart.

They analysed:
– Microclot size & count
– Inflammatory cytokines
– Oxygen uptake
