One of my favorites for this sort of thing was working IT for a restaurant company many years ago. We had a location in Naperville, IL, where we cancelled some phone lines roughly when SBC merged with AT&T. They sent us monthly $0.00 bills for nearly 3 years until someone finally corrected it.
