I'm no expert, but I think there are lots of countries where doctors can require payments over the government rates.

But, even if the US didn't adopt that, I'm not too worried about whether a millionaire doctor has to take a haircut to *only* be upper middle class.

That's already the case in the US. There are tons of doctors that rich people can access that the average Joe can't. In fact, it's worse here than in any country that has universal healthcare. Do you have any evidence showing that it's actually better here? I mean this is just silly.
