There are state structures that increase cost and create barriers to access between you and your meds that wouldn’t exist, such as patents and other IP. The operation gets a lot smaller when its focus is just on the mission -public health- & not creating profits for every rent-seeker in the process

Your meds would be made by a syndicate of worker owned companies. Those companies might be smaller and more regional, but if there’s need and demand society would come together to produce it.
