no worries, it only happened once and it seemed intended especially when the achievement popped up.

i got this one lol. i was doing an early puzzle to get a brass sphere- the one where you click totems to raise/lower some vines, and light 3 orbs quickly. in the process i got killed when standing on top of one of the vines while i raised it.
