You need cross-sectional imaging - specifically MRI

Objective, cross-sectional and safe non-invasive imaging is the key to 1) identifying, 2) staging and 3) monitoring treatment response in fibrostenotic Crohn's.

It's hard to reach with endoscopy, biopsy captures a superficial aspect of the lesion and patient symptoms are often non-specific and modifiable by diet - making patient reported outcomes (PRO) difficult to use.

We've been looking at J-Pouch this week... now for some pictures! On the left is a asymptomatic pouch that was not distended by oral contrast during MRI & the right, a symptomatic pouch which is distended & showing strong contractile behaviour. #IBD

Yesterday we posted about J-pouch - so, why are we talking about this?

At #ECCO25, UCL presented a fascinating study on J-pouch motility, revealing that patients with ileo-anal pouches exhibit higher pouch contractility than controls measured using motility MRI.

Whilst the J-Pouch does not require an external bag and allows patients to regain a sense of normalcy after surgery, the pouch isn't all good. Patients may suffer with pouchitis - inflammation of the pouch lining (affecting up to 50% post-surgery) and also leaks, strictures and/or fistulas.

What happens when someone has their entire colon and rectum removed? 🤔

For those with severe colorectal disease, removing the colon & rectum may be necessary. There are typically 2 options: Ileostomy redirects the ileum to an external stoma, requiring a bag or J-Pouch; an internal reconstruction

How do we identify and measure fistulasing Crohn's diease?

⭐️ MRI is the gold standard for diagnosing and monitoring perianal Crohn's disease, but assessing fistula characteristics can be complex.

Below we have an inactive CIPO bowel (top) and very active CIPO bowel bottom. The heat map quantifies how active/inactive the bowel is. How does this affect management? We're still not sure. But in other areas where we see large, hyper-peristaltic loops GLP1 and GLP2 drugs have been helpful.
