Some attacks will have this danger indicator on the enemies portrait on the turn timeline. Both party members are at full health so I doubt it means that the unit is about to die but it could mean that the attack will drop them to critical HP.

When an enemy is inflicted with break, they move further down the turn timeline, their action is replaced by a weak attack, and they seem to lose their target (likely a random target now).

Several of the animations for the Ruler’s attacks show him transforming his torch and using it to attack the enemy, showcasing his willingness to risk everything for power also seen when he sacrifices his own HP for powerful attacks in gameplay!
