Taken together, our findings suggest a model where common genetic variants predispose individuals to ET, but an additional trigger, such as co-existing rare genetic variants or environmental factors like age or toxic exposures, are needed for ET to manifest.

A secondary analysis comparing the highest and lowest PRS healthy individuals to ET patients revealed key structural differences, namely the VIIIa lobule of the cerebellum being significantly different between either risk group and patients.

Significant associations are found between ET-PRS and grey matter tissue microstructure, particularly in areas like the red nucleus, caudate, putamen, and motor thalamus.

ET-PRS is linked to reduced grey matter volumes in several cortical and subcortical areas, including the cerebellum.

In our work we find that:

Essential Tremor (ET) genetic risk (ET-PRS) shows significant effects on white matter tracts involved in movement control, including mean diffusivity, fractional anisotropy, free water, radial diffusivity, and axial diffusivity.
