A bit of personal news ... over 10 years after being diagnosed with stage 3 testicular cancer, having an orchidectomy, chemotherapy and continual observation (blood tests and chest X-rays), The Christie (Manchester's cancer specialist hospital) have finally discharged me back to my GP. :)

GP action: Please be aware that we have discharged Michael from our GCT follow-up clinic. Please monitor Michael's blood pressure, fast lippid pprofile and glucose for late side effects monitoring. Please also consider to do routine blood tests, including FBC, renal profile, bone profile and liver profile once a year.
Today [11 October 2024],
I have reviewed Michael in Dr Lee's germ cell clinic. Michael remains well in himself, and nil concern on regular TSE.
His tumour marker levels also remain normal, and there is no abnormality reported on his surveillance CXR done today.
As he had completed ten years follow-up post-treatment, I have discussed with Dr Lee who agrees for plan to discharge him back to your care.
I have noted that Michael had diagnosed having T2DM since about five years ago, and he says that his blood sugar levels are currently well-controlled on regular oral anti-diabetic medications.
Could you please continue care for him in the community? And, we would be very grateful if you could do the checks as specified above please. Many thanks for your help.
