As a healer, Shadowbringers ate at Middy. Not being able to heal those who would become sineaters felt like a massive failure. If not for Alisaie's hard work, Middy might not have recovered from it. Because she learned to heal tempering, Middy found the hope they needed to push through Endwalker.
