I believe that there is a bias with prestigious institutions especially for journals that charge you for publication. It is in the best interest of the journal to keep making money if they side with authors from these institutions.

I agree that peer review is needs to be reformed. There are too many opportunities for a biased review and unscholarly conduct. In particular, this seems to be an issue with megajournals with a single-tier editorial board.
