That'd also explain why she went so hard on him specifically and ended up killing him when she previously simply attacked her targets without fatal damage being done.Personal feelings got involved and she went too far. We do see signs of her possibly being triggered before, during and after the kill

I personally believe that milgram might have intentionally made the two men look similar since Kotoko canonically has the tendency to project heavily. So she could have started to associate Kaneshiro with the man that hurt her due to their similarities in appearance and cases.

Kotoko didn't save anyone else other than that girl and when she stormed in the storage room, the guy was with her, not doing anything else. So Kotoko was most definitely present when the things shown in the flashbacks were shown.

I've thought about that too but how would Kotoko have these exact images in her memory? They're very specific, with his shadow showing, him swinging a hammer, the girl on the floor next to the hammer. Those are most definitely memories and not images shown in news.
