The harms are serious: suicide, homicide, and psychological distress. Those most affected included people diagnosed with a personality disorder, people who self-harm, and those labelled “hard to engage.”

Our scoping review is the first of its kind in the UK. We found institutional neglect was the most common issue—often through gaps in access to care.
