Contributing factors spanned national policies, institutional resources, and everyday practices. Current safeguarding & patient safety policies don’t fully address these risks in community settings.

The harms are serious: suicide, homicide, and psychological distress. Those most affected included people diagnosed with a personality disorder, people who self-harm, and those labelled “hard to engage.”
