I've not actually modded Fallout 3 myself, but from what I can tell from looking around, most people recommend doing a Tale of Two Wastelands setup. New Vegas apparently has a more stable engine.
I've been playing Fallout 3 vanilla and the only thing I find myself needing/wanting are Iron Sights.
