I don't think they ever explains it, but it's assumed that since Buffy already died in the season 1 finale she's already been replaced by Kendra and then Faith replaced Kendra.

A lot. Ripper is still in there, but mostly I think he knows it must be done and he'd rather do it himself than put the burdon on Buffy. I think it's the right decision, he is far more capable of carrying the guilt of it.

The show wasn't renewed for the last two seasons yet when this was written, if I remember correctly, so it was written as a series finale. I think by the time they filmed it they had been renewed.
