Nitrofurantoin is still first-line for uUTI (definitely lots of love for NTF! Although we recommend 5 days - would love to see 3d outcomes published!); the US guidelines are just starting to explore cephalosporins (previously alt agents) - now working to get data out on which ones to use vs. avoid!

We use cephalexin 500 BID for uUTI and 1,000 mg BID for cUTI (unless bacteremic, or obese ➡️ then 1,000 mg TID-QID depending on BMI); academic.oup.com/ofid/article...
