I was a computer programmer for Blue Cross Blue Shield in the 1970’s. I saw the actual code that rejected claims for no reason. Was told “saves $, most people don’t appeal”. I was outraged, quit that job. So this is not new; The health insurance industry has used these tactics for over 50 years.
