Absolutely true. And I could certainly posit a case where I would defend my use of a spinal. But I have given testimony in enough malpractice cases to know that good reasoning isn’t always enough to protect you.

In our trauma center, the patients are usually started on heparin when admitted. Much easier to deal with than longer acting agents. An NOF will be in the OR within 48 hours unless medically contra-indicated

Fentanyl- yes. Alfentanil is no longer available in the U.S. A common tiva combo is propofol-remi-rocuronium

Most people do not dream under general anesthesia because, unlike sleep, there is no REM stage. Under sedation (rather than GA), dreams can occur

Skin necrosis was a rare but serious complication that could occur if thio was injected into the soft tissues instead of the vein. Almost always through a dislodged or misplaced IV. I never heard of anyone doubling the thio conc, but sooner or later people do anything

I’ve seen things given into an IA catheter, an epidural catheter and even had a disaster when something was given into a peritoneal catheter. Murphy’s Law always applies.

I’ve always hated vomidate, both as an anesthesiologist and a patient. But when you have no other reasonable induction agent for 4 months, you live with it.
