There was an effort to get insurers and hospitals to cough up master prices lists for the public but these days HHS is more focused on fatshaming and undermining vaccines than making the health system work.

It was even a Trump idea, from his first term! One of the only constructive things he did!

We already have the GOP's preferred health plan, but they branded it too strongly as Obamacare and now their base hates the name and depends on the services.

He's at least setting up the conditions to fight back, even if the effort might not ultimately succeed. There don't seem to be many other people with power handing out crowbars to other institutions so they can try and pry fascism out of the system.

Which is sort of what the PPACA did for the marketplace and individual consumers/freelancers. A lot of employers and carriers are already naturally bolted on to those systems anyway just for UX benefits, even!

However, the GOP is ideologically dedicated to killing Obamacare, so.

Both programs are thoroughly infiltrated with private actors already but it forces more and more of the overall market into federal regulation and oversight, and ensures you've always got an *option* at least. Leaves inmigrants uninsured but that's politically toxic 😭.

I've long thought that the quickest way to get universal healthcare was "drop the Medicare enrollment age by 1 every year and grant universal Medicaid eligibility to children then add one year of eligibility to Medicaid until they meet in the middle."

But even in the US there was much more public support for lockdowns than the information environment suggested; the political imperative to reopen schools and barber shops in autumn 2020 actually cut strongly against public opinion, even into late 2021 *after* vaccines were deployed.

Every other advanced economy put simpler, cheaper, and easier systems in place to solve the universal coverage problem, but they were dead on arrival here for political reasons. Which have led us close to where we started on healthcare before the PPACA, with a coverage crisis and rising costs.

Compared to the scale of what was required, the PPACA was simply not ambitious. It wasa complex fix over two crises - rising "rate of change" for healthcare costs and shrinking coverage that was going to provoke insurance death spirals.
