My favorite puzzle/dungeon was a dwarven tomb taken over by a Lich. She kept rearranging the tomb, and no matter what they did, she was taking no real physical damage. Of course, the key was that the tomb itself was turned into her phylactery. I was very proud of them for figuring it out.

Karthus has this old trinket his adoptive sister made for him. It broke years ago, but he can't bring himself to let it go since he thought she passed on. Turns out, he found her 6 months later and joined her adventuring party, getting his trinket fixed in the process.

When the Break occured and extraplanar creatures began to cross over, it was signified by the falling of stars across the land. Two of these stars turned out to be fallen deities, but the rest crashed to the earth. People began quickly hunting these fallen stars down for a new power source.
