It is an edit of the "can an ambulance" meme, but adjusted for a specific purpose that is (intentionally) not made clear in this image. The comedy is derived from the absurdist shift from reversing the attacker's intent, to making a joke about self-harm. Sometimes joking about self-harm can be very bad, but absurdism can actually help people cope with feelings of sorrow or grief. The comedy and dismissal of any serious undertone may help defang the feelings depression, robbing them of their power.
