It’s hard to undervalue the social process of getting an idea off the ground in designing education.

A proposal doesn’t appear out of the blue and gets approved. Meetings before the meetings are critical to the iterative process to go from idea 1.0 to the final version.

One key ingredient for good assessment is attention. While time is of course necessary, I think that when educators say time is a limitation for them, they also implicitly mean attention.

This requires us to develop both individual and systems level management of attention for better learning.

The data for enrollment to med school since the Supreme Court's aff. action decision shows Black enrollees down by 11.6% compared to last year and Latinx down by 10.8%. Even steeper drops for American Indian/ Alaska Natives - down by 22.1%, NHPI down by 4.3%.

Something we have to do in CBME/feedback in the clinical setting is both recognize all the immense hard work that happened and that the learner still has important things to improve.

Acknowledge the immensity of effort caring for patients. And coach for improvement.

If fundamental to effective teaching is the fostering of trusting educational connection and co-creation between teacher and learner, then we must design for the factors required.

Time, clarity of purpose, available cognitive load, mastery of emotional intelligence to name a few.

The coming years will see the automation of assessment with the use of AI. Goals for volume of evidence in CBME will be met in part by the speed and productivity boost of these tools.

Another story will be how teaching and in-person assessment become bespoke and a craft service.

AI and the use of tech in assessment may help with productivity and speed. But I think it is worth thinking about assessment especially in the clinical environment also as a relational activity between a teacher and learner co-creating meaning to foster learning.

I think a fundamental ingredient to CBME is teachers providing concentrated cognition to coach and assess in the clinical environment. This kind of undivided, one-on-one attention is invariably costly. But I truly believe it to be worthwhile.
