A standing stone that was an information-ivore which emerged at the Big Bang, and got trapped as the Earth formed. It recognised the PCs were PCs and started communicating with the players (and I hid little rocks in the players' pockets and dice bags). One player had nightmares for weeks.

A novelist dreamed up a character so completely that she became pregnant in the brain with the foetus. Massively distended skull. She gave birth by breaking her own jaw.

I've tried a few and dislike the form. They're adjacent to RPGs. But as they only record the performed actions of characters and can't show the internal thinking of a player, I find them hollow and trite.
