I think the significance is that metformin remains an effective diabetes medication whereas SGLT2i are considerably less effective in advanced CKD. There was a Taiwan retrospective registry study finding increased mortality in patients on metformin with advanced ckd

The STEP study, used less than 130 as a treatment arm and showed similar benefits to SPRINT. However, the usual treatment arm allowed was less than 150.
